Hyaluronic Acid vs. Biostimulatory Fillers: What's the Difference?

Most of the popular fillers on the market today contain hyaluronic acid (HA), a substance that is naturally found in our skin. HA is very hydrophilic, which means it attracts and holds onto water. HA fillers add volume by physically filling in the area where they are injected, as well as by drawing water into the area to enhance the filling effect.

One interesting and useful property of HA fillers is that they can be dissolved to minimize or even reverse the effect if desired. This is usually unnecessary, but it is nice to have this option if needed.

Biostimulatory fillers contain various other active ingredients that trigger the skin’s natural regenerative process. In addition to physically adding volume to the area, they gradually restore facial fullness by replenishing lost collagen. These fillers tend to last longer than HA fillers because the new collagen remains in the body after the product breaks down.

The downsides are that biostimulatory fillers take longer for onset than HA fillers and they cannot be dissolved.

How Should I Prepare for Dermal Fillers?

Please take the following steps before your filler appointment:

Two weeks prior:

  • Avoid dental work, including routine dental cleanings
  • Avoid vaccines
  • Avoid infection/illness (inform your injector if you are not feeling well)

One week prior:

  • Avoid blood thinning medications and supplements (aspirin, ibuprofen, Aleve, Vitamin E, fish oil)
  • Start Arnica to help with bruising (this is not required, but can improve your post-treatment experience)

One day prior:

  • Avoid alcohol

What Should I Expect During My Filler Treatment?

Your consultation may take place immediately before the procedure, but you will never be rushed. This is your opportunity to discuss your aesthetic concerns and learn more about the treatment options available to you in a relaxed and receptive environment.

Before proceeding with the injections, your provider may apply a topical anesthetic to ensure your comfort with the aid of the Accuvein light source. They will then administer the filler beneath the skin using a fine needle or cannula. You may feel a slight pinch or stinging feeling during the process, but overall, the treatment is relatively painless. Your injections will take place with the aid of a light source that will illuminate your blood vessels to reduce the chance of bruising.

Fillers can typically be administered in 15 to 45 minutes, after which patients are free to resume their regular routine.

What Is the Downtime for Dermal Fillers?

Dermal fillers are one of the most convenient rejuvenation treatments available. You may return to most activities right away and side effects are typically mild, if they occur. Some patients experience mild redness, swelling, bruising and/or tenderness at the injection sites. There is no need to take time off work or your social life as long as you are comfortable with your appearance. We provide these instructions for your safety and for the best aesthetic outcome. Please adhere to the following post-treatment guidelines:

  • Avoid makeup, touching the injected area, exercise and saunas for 24 hours
  • Ice swelling to reduce puffiness and discomfort
  • Avoid dental appointments and vaccines for two weeks
  • Inform us of any significant bruising — certain devices we have in-office can help bruises resolve faster
  • Contact us immediately if you experience abnormal blotching/changes to your skin color, severe pain, fever, redness/heat to the skin

How Long Does It Take to See Results From Fillers?

With most fillers, an increase in fullness and/or definition in the treated area is seen right away. Keep in mind that the area may initially look overly full due to swelling. This swelling typically fades within a few days, and the filler will settle in about two weeks. This is when you can assess your final result.

How Long Do Dermal Fillers Last?

The longevity of your dermal fillers will depend on which product we use, where it’s injected and your natural metabolism. Over time, the product will naturally break down, and the area will return to its pre-treatment appearance.

Most hyaluronic acid fillers last six to 18 months. Biostimulatory fillers can last longer, up to two years or more in some cases. Regular treatments will help maintain your results.

What is the difference between dermal fillers and Botox?

Fillers are injected under the skin to restore lost volume, smooth lines or enhance facial contours. When we use fillers to reduce wrinkles, we are specifically targeting static wrinkles — the type of line you can see when your face is at rest.

Botox and other neurotoxins are injected into muscles to reduce muscle movement. This targets dynamic wrinkles — the type of line you see when your muscles contract to make an expression like frowning, squinting or smiling.

Both treatments are effective, minimally invasive and can be part of a comprehensive approach to facial rejuvenation.

Can dermal fillers be reversed or removed?

Fillers made with hyaluronic acid can indeed be reversed or removed. This is one of the key advantages of HA fillers – they offer a safety net, so to speak.

If for any reason you’re not satisfied with the results, or if there’s a need to reverse the filler due to a complication, an enzyme called hyaluronidase can be used. This enzyme acts quickly to dissolve the hyaluronic acid in the filler, reversing its effects. The process is relatively simple and can be performed in our office.
